School District 181 Opt Out Form

Instructions for Completing the D181 Opt-Out Request

(Use these instructions when filling out the official D181 Request to Opt Out of Curriculum Content form.)

Part I
• Review the list of sexualized books identified by Concerned Parents in your child’s D181 school.
• As required by D181 policy, please read each book in question to determine if you also deem it age-inappropriate for your child.
• In addition, list any other books you have discovered in your child’s school that you believe are not appropriate.
• Please also include any lessons, classroom activities, or special programs you object to.

Part II
• After identifying the materials in Part I, check the corresponding box under Curriculum and Instructional Materials Exemptions.
• In most cases, parents should check the second box, which reads:
“Comprehensive Personal Health and Safety Education and Comprehensive Sexual Health Education.”
• Be sure your Part II selection aligns with the books, lessons, or programs you listed in Part I.

Part III
• If your objection is based on religious beliefs, clearly state the specific belief that forms the basis for your request.
• Explain how the instructional material, lesson, or program conflicts with or places a burden on your religious beliefs.
• Example: “LGBTQ content is against my family’s Christian beliefs.”

Final Steps
• Fill in your D181 student’s information and your parent/guardian information.
• Send your completed opt-out request directly to your child’s D181 principal.
• Keep a copy of the signed form for your records.
